PROCEDURE

Create a Tab with 1mm thickness and and use the cutout botton option to remove the left side of the tab of 5mm wide and 40mm long. At 10mm distance from the edges bend the tab inside
Add Flanges to the corners and break the corners of the flanges by using break corner option
Now Unbend the part including flanges and add holes to flanges with diameter 2.4mm
Add dimples to the centre and corners of the tab as per given dimensions
Now re-bend all the surfaces and make a part as shown in figure
